Please follow these instructions and ship your radio to the address below:

  1. Verify that your radio is listed in the models we support.
  2. Fill out and print a Bose Repair Form below. This form is provided for your convenience to print and include with your Bose radio when shipping it to us for repair. Optionally, you can just print the Bose Repair Form PDF.
  3. Pack the radio generously with padding or bubble wrap. Many radios are damaged easily without it.
  4. Send the radio and form to:

    We will only bill you when we recieve the radio.

Ackknowledgement: I have read and understand that the estimate and terms given, and authorize repair of the above described equipment for purposes stated. I also understand when authorized repairs along with necessary materials are completed and express repairmen's lien is hereby acknowledged on above set to secure the amount of repair thereto. Repairs left over 90 days of notification will be SOLD for parts and labor.
